Welcome to Midwest Primary Care web page and thank you for taking an interest in your prostate health. At Midwest Primary Care, we are working with Little Company of Mary to create a pain-free experience for patients. Our office is conveniently located next to the hospital in the Mary Potter Physicians Pavilion at 2850 West 95th Street, Suite 106. A pharmacy in our building provides convenience for our patients to see us and then, if necessary, fill a prescription.

Dr. Thomas has been a physician for 26 years and has been on staff with Little Company of Mary for 19 years. He is a graduate of Kansas City College of Osteopathic Medicine and completed his residency at Chicago Osteopathic/ Midwestern University. Dr. Thomas is married with four children. When it comes to his patients, Dr. Thomas likes to help make people feel good. In fact, his patients usually say he is easy to talk to. His medical specialty is geriatrics and, in his spare time, he enjoys baseball and fishing. |

 | | Andrew Perez, M.D.
Dr. Perez has been a physician at Little Company of Mary for 22 years. A graduate of the University of Illinois School of Medicine, Dr. Perez completed his residency on the north side of Chicago at Illinois Masonic Medical Center. Dr. Perez is married with two children. His treatment philosophy is to treat each patient with compassion. He tries to be a good listener and honest when it comes to discussing the health of his patients. His patients think he is friendly, funny, and someone with a great memory. In his free time, Dr. Perez enjoys playing tennis. |

Dr. Mohsin has been a physician for 12 years and has been on staff at Little Company of Mary for the past six years. He is a graduate of Bangalore University, India, and he completed his residency and fellowship in geriatrics at Loyola University Medical Center. Dr. Mohsin is married with three children. As a physician, Dr. Mohsin's main goal is to improve the quality of life and well-being for his patients and for the community. Dr. Mohsin's patients think he is easily approachable and genuinely cares about their problems. Dr. Mohsin enjoys traveling. |
